New outbreaks of the lethal H5N1 strain of the avian influenza have been documented in Vietnam and China. According to a Vietnamese animal health officer, 4,000 poultry and water fowl have died of the virus after October 25 in provinces 70 kilometres north of Hanoi. In China, 9,000 chickens and twenty wild birds have died of the virus in just over two weeks with more than 300,000 birds culled as a precaution.

Meanwhile, Japanese authorities are planning on culling 180,000 chickens in response to the discovery of the first case of bird flu in the country in over a year. Most of Japan’s previous avian influenza outbreaks have been due to the H5N2 strain, which poses no known threat to humans. The virus found in a farm north-east of Tokyo has been confirmed as belonging to the H5 family, but additional tests are needed to find out whether it is the H5N1 strain. A Japanese agriculture ministry official said that there was “little concern” of the virus being the H5N1 type as no case of the strain has previously been found in the area.

At least 60 people have died of the H5N1 strain in east Asia. Over 40 of the fatalities have occurred in Vietnam, where tests are currently being conducted for at least one possible additional human infection. China has not reported any human infections of the avian flu.

Babies in neonatal intensive care units are sensitive to hospital infections. The causative bacterium in these cases in Spain, Klebsiella pneumoniae, does not usually cause problems in healthy individuals. It’s an opportunistic pathogen, which means it usually only causes disease in the weakened and sick.

A Spanish hospital reported today that three premature babies have died within one week from the same infection. The babies, who were being cared for in the neonatal intensive care unit of Madrid‘s 12 de Octubre Hospital, all died after being infected with Klebsiella pneumoniae bacterium. Authorities also suspect that a fourth baby has become infected with the bacterium. The Spanish patients’ association ADEPA has asked the public prosecutor of the Madrid Region, Manuel Moix, to open an investigation into the matter.

This morning, hospital authorities stated that the first death occurred on 26 February. A baby born eight weeks prematurely and weighing just under one kilo died due to septic shock arising from the infection, which was unconnected with the symptoms of his premature birth, according to hospital reports.

Shortly afterwards, two babies in the same unit died on 2 March. They appear to have died from the same infection; however the hospital stated that they are still waiting for confirmation of the cause of death. A fourth baby has been found to have the bacteria on his skin, but as yet there is no confirmation of whether he has been infected. The hospital states that although the baby is in a serious condition, this is to be expected when a baby is born so premature, and that his condition is also due to other factors, unrelated to the bacterium, which have arisen since his birth.

The hospital authorities have asserted that all of the babies in its care are under close supervision, and are being periodically monitored by the Preventive Medicine Centre, in order that the most up-to-date information may be had on the state of any and all patients in the hospital.

To prevent further infections, the hospital has taken several measures. The first is of course rigorous medical hygiene. High-risk patients are referred to other hospitals. The hospital has been divided into two separate zones, one for those already infected and the other for those who have not yet been affected. As a consequence, 25 children who were staying in the hospital while the 3 premature babies died, remain separate from the others, to make sure newly admitted children are not exposed. The nursing staff has also been restricted to one or other of these two areas. Visits from specialists of other hospital services, and from families, has been restricted. Furthermore, an epidemiological study is being undertaken to determine the origin of the situation.

The newspaper El País reports that the bacterium Klebsiella pneumoniae is to be found in hospitals as a matter of course and is often contracted by those who have been in hospital for prolonged periods (termed a nosocomial infection). The mortality rate from the bacterium is extremely high in those cases where it’s contracted by people with serious illnesses, as is often to be expected in the case of a premature birth, which can result in newborn babies weighing less than 500 grammes.

According to the Spanish daily, this type of bacterium is prevalent in hospitals throughout the world, but the incidence of infection in Madrid hospitals is lower than the Spanish average. The head of the Neonatal Unit at 12 de Octubre Hospital, Carmen Payás, explained that the bacterium is very adaptable “and keeps on learning”. The father of the dead baby, an Ecuadorian named Angel Marcelo, was quoted as saying that the progress of the baby had at first been “tremendous”, and that he had even been taken off the respirator, but that a few days later he began to cough up blood, dying soon afterwards.

The chair of ADEPA, Carmen Flores, has appealed to the Madrid public prosecutor to open an investigation into the situation and to find those responsible. ADEPA have suggested that the investigation centre on the number of casualties among infants in the hospital, and on the question of whether conditions in the neonatal unit were a direct contributor to the incidence of the infection.

José Quero, head of the Neonatology Department of the La Paz hospital in Madrid said that, “sadly enough”, this situation was “not something exceptional,” but rather something neonatologists have to watch out for.

The Iranian police forces have faced criticism from Ayatollah Hashemi Shahrudi, the head of the judiciary who was appointed by Grand Ayatollah Ali Khamenei, for their re-invigorated campaign to do away with un-Islamic dress.

Ayatollah Shahroudi proclaimed, “Tough measures on social problems will backfire and have counter-productive effects.” Others have, of course, made it clear that un-Islamic dress can lead to moral corruption, engender innumerable vices, and hurt the Islamic character of the nation.

Some believe that no one had any issue with the creation of an Islamic atmosphere. The core of the matter revolves around the implementation of the Islamic dress code; additionally, heavy-handed measures should be shunned. For instance, Mehdi Ahmadi, information head of Tehran’s police, told Al Jazeera: “Some citizens may complain about the way the law is being enforced but they all agree with the plan itself.”

Following the Islamic Revolution in 1979, hijab became mandatory in Iran for every woman including foreigners after over 98% of citizens voted for an Islamic government. Women may face caning up to 74 strokes for failing to observe hijab. In this recent crackdown, the authorities have arrested many citizens throughout the country. Not only have women been taken into custody for their hair being uncovered on their foreheads and tight clothes that show body shapes, For men they need to cover from knee to their waist as according to Sharia. Even a foreign journalist was detained because the photograph on her press card was indecent.

It has not been clear whence the directive for the re-newed clampdown emanated. Some have blamed Mahmoud Ahmadinejad while Gholam Hossein Elham, the government spokesman, stated to reporters, “The police work as agents of the judiciary to confront crimes. The government as an executive body does not interfere in the affairs of the judiciary.” The following pre-election speech seems to corroborate this latter statement:

In reality, is the problem of our people the shape of the hair of our children? Let our children arrange their hair any way they wish. It doesn’t concern me and you. Let you and me overhaul the basic problems of the nation. The government should fix the economy of the nation and improve its atmosphere…[It should] better psychological security and support the people. People have variegated tastes. As if now the arch obstacle of our nation is the arrangement of our kids’ hair and the government disallowing them <He chuckles>. Is this the government’s responsibility? Is this the people’s merit? In actuality, this is the denigration of our people. Why do you underestimate and belittle the people? It is the real issue of our nation that one of our daughters donned a certain dress? The JLG factory announced in April 2009 that it will fire 53 of its 163 workers by the end of 2009, while the remaining 110 jobs will not be secure over the next 2 years.

JLG Tonneins was acquired in 2006 with its parent JLG Industries, a maker of aerial work platforms, by the U.S.-based Oshkosh Corporation. Despite being hugely profitable in the past, production has been much reduced since 2008 with the contraction of the construction industry and lower demand for its products. Despite excellent past results the new American management demanded sweeping cuts at the company.

In the view of locals, “the company’s actions are a disgrace given the expensive perks, such as official cars, for its corporate fat cats, compared to the sacrifice, silence, and dignity demanded by the company of those it has made redundant.”

The management offered severance pay of 3,000 (US $4,200), however the workers demanded a severance package commensurate with “the wealth that their labor has generated.” Worker’s delegates requested a “supra-legal” payment of € 30,000, on Thursday 16 of July the management responded with a counter offer of € 16,000. On Thursday night the worker’s actions secured the € 30,000 settlement initially demanded.

Nepal Parliament passes resolution to curb King’s power

Thursday, May 18, 2006Prime Minister of Nepal, Giriraj Koirala proposed in Parliament a resolution which is aimed at drastically curtailing the monarch’s powers. According to the resolution, the King will be stripped of his status as the Supreme Commander of the Royal Nepal Army (which is to be renamed as the Nepal Army Cabinet). Portions of the Nepalese national anthem that praise the King have been cut.

The proposal also aims at cutting down on the King’s allowance and his right to be exempted from paying taxes. The government which is currently referred to as the “King’s administration” will henceforth be known as the “Nepalese Government”. The resolution also changes Nepal’s status from that of a Hindu nation to a secular one. The King’s Advisory Council will no longer exist and his security will be taken care of by Parliament. The King will also now no longer have the privilege of being above the law of the land since the resolution provides for him to be tried in court if the situation so warrants.

Analysts have expressed concerns saying that under the current Constitution, this proposal cannot become law till the King signs it. Politicians say however that this proposal is above the Constitution and reflects the will of the people. King Gyanendra restored democracy to the Himalayan Kingdom after weeks of massive anti-monarchy protests earlier this year.

A robotic system at Stanford Medical Center was used to perform a laparoscopic gastric bypass surgery successfully with a theoretically similar rate of complications to that seen in standard operations. However, as there were only 10 people in the experimental group (and another 10 in the control group), this is not a statistically significant sample.

If this surgical procedure is as successful in large-scale studies, it may lead the way for the use of robotic surgery in even more delicate procedures, such as heart surgery. Note that this is not a fully automated system, as a human doctor controls the operation via remote control. Laparoscopic gastric bypass surgery is a treatment for obesity.

There were concerns that doctors, in the future, might only be trained in the remote control procedure. Ronald G. Latimer, M.D., of Santa Barbara, CA, warned “The fact that surgeons may have to open the patient or might actually need to revert to standard laparoscopic techniques demands that this basic training be a requirement before a robot is purchased. Robots do malfunction, so a backup system is imperative. We should not be seduced to buy this instrument to train surgeons if they are not able to do the primary operations themselves.”

There are precedents for just such a problem occurring. A previous “new technology”, the electrocardiogram (ECG), has lead to a lack of basic education on the older technology, the stethoscope.
